SPIN and PSWQ: two short scales that sharpen your anxiety differential

The GAD-7 is a strong general anxiety screener, but a mid-range GAD-7 hides two very different clinical pictures: generalized worry and social anxiety. Adding two short scales — the PSWQ and the SPIN — sharpens the differential and changes what you plan to do in treatment.

PSWQ — the worry profile

Sixteen items, all about pathological worry: uncontrollability, pervasiveness, and duration. A PSWQ ≥60 is highly suggestive of GAD, and the scale is unusually good at distinguishing GAD-type worry from the situational anxiety seen in adjustment disorders. It's also more sensitive than the GAD-7 to change under specific worry-focused interventions (Metacognitive Therapy, worry-postponement, intolerance-of-uncertainty protocols).

SPIN — the social anxiety profile

Seventeen items covering fear, avoidance and physiological reactivity in social contexts. A SPIN ≥19 is a clinically meaningful cut-off; ≥31 suggests severe social anxiety disorder. The SPIN discriminates well against panic disorder and GAD, and its subdomains (fear, avoidance, physiology) tell you where to intervene first — pure avoidance-driven presentations respond fast to exposure hierarchies, while high-physiology profiles often need arousal-management work before exposure lands.

Why not just use the GAD-7?

The GAD-7 asks about symptoms of anxiety in general, not their focus. A patient scoring 14 on the GAD-7 could be an intensely worrying generalized-anxiety patient, a socially anxious patient whose anxiety spikes at every meeting, or a health-anxious patient. All three score similarly on the GAD-7. The treatment plans are different. Ten minutes of PSWQ + SPIN saves weeks of misdirected work.

• PSWQ — worry-specific severity and treatment target.
• SPIN — social anxiety-specific severity, avoidance, physiology.
