Screening checklist for signs of dyslexia in children and adolescents (ages 6–16). It should be completed by an adult who knows the child well at home or at school (parent, guardian or teacher). Indicate how often you have observed each behaviour over the last 6 months, compared with what would be expected for their age and school year. This is not a diagnostic test: the result indicates whether a formal literacy assessment is needed.
