Childhood Trauma Questionnaire — Short Form (CTQ-SF). The following statements refer to experiences you may have had while growing up. Although some are personal in nature, please answer as honestly as you can, indicating how often each situation happened. Retrospective screening instrument: it is not a diagnosis nor forensic evidence on its own.
