The EDAH is a rating scale designed to be completed by the primary-school teacher. It screens for the four ADHD-related dimensions: hyperactivity, attention deficit, conduct disorder and oppositional-defiant behavior. Rate how frequently each behavior has been observed in the classroom over the past weeks.
1.Has excessive motor restlessness.
2.Has emotional outbursts and impulsive reactions.
3.Bothers other children frequently.
4.Is easily distracted, has short attention span.
5.Changes activities frequently.
6.His/her demands must be met immediately, becomes frustrated easily.
7.Cries often and easily.
8.Sudden and marked mood changes.
9.Behaves in a defiant or hostile way toward the teacher.
10.Refuses to obey instructions and challenges authority.
11.Argues and quarrels with peers.
12.Displays negative, sulky attitudes.
13.Blames others for his/her own mistakes.
14.Fails to finish the tasks he/she starts.
15.Has difficulty maintaining attention on tasks.
16.Overall academic performance is poor.
17.Has a low tolerance for frustration.
18.Tells lies, cheats, or steals.
19.Is aggressive, verbally or physically.
20.Denies having done things that are attributed to him/her.
